چکیده

PURPOSE: The ideal monitoring tool to evaluate free flap success should be minimally invasive, continuous, costeffective and reliable. Our group has previously introduced implantable oxygen sensors as a mean to monitor flaps in the immediate post-operative period and detect acute vascular compromise. The purpose of the current study was to compare and contrast intradermal vs. subcutaneous implantation of the sensors in their ability to detect flap compromise.

Perfusion and Oxygenation of Random Advancement Skin Flaps Depend More on the Length and Thickness of the Flap Than on the Width to Length Ratio

OBJECTIVE The aim was to investigate the relationship between the dimensions (length, width, and thickness) of random advancement skin flaps and retained tissue perfusion and oxygenation. METHODS Flaps were raised on the flanks of pigs. The flaps were either 0.5 or 1.0 cm wide, thin (dissected halfway through the subcutaneous tissue) or thick (dissected down to the muscle fascia). Tissue perf...

Abstract: Meshed Acellular Dermal Matrix with Overlying Split Thickness Skin Graft for Scalp Reconstruction

RESULTS: Following a normal, spontaneous vaginal delivery, the patient underwent bone grafts and ORIF of the left forearm and metacarpal bones. We then utilized the newly available and pliable abdominal skin to cover the large soft tissue defect. The abdominal wall flap was raised, thinned and then the injured extremity was carefully inset in a subcutaneous abdominal pocket and a meshed bilayer...

Testing a device to replace the leech for treating venous congestion.

OBJECTIVE To test the effectiveness of a device designed to promote decongestion and tissue survival of a fasciocutaneous flap during 15 hours of complete venous obstruction. METHODS In a porcine model, a 9 x 7-cm fasciocutaneous flap was elevated and the associated veins were clamped, causing complete venous obstruction for 15 hours in 6 control and 6 treatment animals. Up to 3 devices were ...

The use of visible light spectroscopy to measure tissue oxygenation in free flap reconstruction.

The loss of a free flap is a feared complication for both the surgeon and the patient. Early recognition of vascular compromise has been shown to provide the best chance for flap salvage. The ideal monitoring technique for perioperative free flap ischemia would be noninvasive, continuous, and reliable. Visible light spectroscopy (VLS) was evaluated as a new method for predicting ischemia in mic...
